Description : Read Now Trimodal Therapy For Invasive Bladder Cancer by and you can download with pub, pdf, txt, doc, and more file format with free account. abstract : purpose of review: trimodal therapy (tmt) is considered the most effective bladder-sparing approach for muscle-invasive urothelial carcinoma of the bladder (mibc) and an alternative to radical cystectomy. the purpose of this article was to review and summarize the current knowledge on the equivalence of tmt and radical cystectomy based on the recent literature. recent findings: tmt consists of a maximal transuretral resection of the bladder, followed by a concurrent radiotherapy and chemotherapy, limiting salvage radical cystectomy to nonresponder tumors or muscle-invasive recurrence. in large population studies, less than 6% of the patients with nonmetastatic mibc receive a chemoradiation therapy and this rate is stable. a growing body of evidence exists that tmt provides good oncologic outcomes with low morbidity when compared with radical cystectomy. tmt requires, however, a close follow-up because of the high risk of local recurrence and salvage radical cystectomy in up to 30% of the patients. salvage radical cystectomy can be performed with adequate results but does not offer the same opportunity of reconstruction and functional outcomes than primary radical cystectomy. summary: although radical cystectomy is still the treatment of reference for most of the patients with localized mibc, tmt represents a reasonable alternative in highly selected patients. any firm conclusion on the equivalence or superiority of one treatment to the other is still limited by the lack of randomized controlled trials and the heterogeneity of the available literature. future studies and multidisciplinary approach are mandatory to optimize the patient selection and regimen of tmt.

Description : Read Now Prediction Models For Progression Of Non Muscle Invasive Bladder Cancer A Review by and you can download with pub, pdf, txt, doc, and more file format with free account. abstract: an accurate prediction of progression is critically important in the management of non‐muscle‐invasive bladder cancer. at present, three risk models are widely known for prediction of the risk of tumor recurrence and progression of non‐muscle‐invasive bladder cancer: the european organization for research and treatment of cancer, club urológico español de tratamiento oncológico, and new european organization for research and treatment of cancer models. bladder neck involvement has been shown to be one of the significant predictors for progression in non‐muscle‐invasive bladder cancer, and a new scoring model (tokyo medical and dental university model) consisting of bladder neck involvement, tumor grade, and stage has been developed and externally validated. however, the predictive abilities of these models are still unsatisfactory, and more precise models are necessary for accurate individual prediction of prognosis. until now, time‐fixed analysis has been used for most studies predicting the prognosis and outcome of non‐muscle‐invasive bladder cancer patients. in order to predict progression more precisely, time‐dependent models should be developed using multiple‐event analytical techniques, as non‐muscle‐invasive bladder cancer often progresses to muscle‐invasive bladder cancer after multiple recurrences and changes in tumor characteristics over a long natural history. integration of molecular markers is also a promising approach. a validated model that accurately predicts the risk of progression would help urologists and patients decide whether and when to choose radical cystectomy on an individual basis.

Description : Read Now Genetic Subtypes Of Invasive Bladder Cancer by and you can download with pub, pdf, txt, doc, and more file format with free account. abstract : purpose of review: recently completed cancer genomics projects identified intrinsic subtypes in muscle-invasive bladder cancers. here we will describe the studies that led to their discovery and review their biological and clinical properties. recent findings: whole genome mrna expression profiling and unsupervised hierarchical cluster analyses identified intrinsic basal and luminal subtypes in muscle-invasive bladder cancers that are similar to the ones found in breast cancer. tumors within each subtype have distinct responses to conventional cisplatin-based combination chemotherapy, and they contain gene expression signatures and dna alterations that may render them vulnerable to clinically available targeted therapies. summary: like their breast cancer counterparts, basal bladder cancers are characterized by poor clinical outcomes in the absence of effective systemic therapy, but a large fraction of them do respond to neoadjuvant chemotherapy, suggesting that the tumors should be managed aggressively. on the contrary, tumors that belong to the 'p53-like' subtype tend to be chemoresistant, so patients with these tumors should probably be managed differently. it seems likely that prospective identification of tumor intrinsic subtype membership could complement the use of dna-based biomarkers to identify the groups of patients who will benefit the most from chemotherapy and targeted agents.

Description : Read Now Deceptive Muscle Invasive Bladder Cancer Recurrence With Benign Biopsy Foci After Bladder Sparing Treatment by and you can download with pub, pdf, txt, doc, and more file format with free account. abstract abstract most of recurrent bladder carcinoma after partial cystectomy did not cause diagnostic difficulties for urologists, because of the appearance of typical papillary in ultrasonography or cystoscopy, and could be easily confirmed by tumor biopsy. three patients, ages from 35 to 62 years, had undergone bladder sparing treatment for muscle invasive bladder cancer, all of them had biopsy revealed benign bladder lesion at surveillance cystoscopy. however, transurethral resection of bladder tumor showed high-grade muscle invasive urothelial bladder carcinoma for these patients. two patients were thus delayed for timely cystectomy and consequently resulted in local or distal metastasis. as a result, we recommended that timely pelvic enhanced computed tomography and transurethral resection of bladder tumor were necessary when bladder lesion occurred after partial cystectomy, avoiding the possibility of missing muscle invasive urothelial bladder carcinoma recurrence and delaying timely cystectomy.
